POSITION SUMMARY:
The Intake Specialist is a full time staff position serving as the initial point of contact for individuals and families entering the SSVF program. The Intake Specialist conducts the SSVF assessment, identifies program eligibility, prepares client files, and facilitates referral to case management.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Program Duties:
- Manages a high volume of intakes while maintaining high levels of customer satisfaction and data quality.
- Conducts comprehensive intake with individuals and families to document: housing status, income, non-cash benefits, current needs, household information, and all other required information.
- Maintains fluency in admission requirements for participating projects to ensure accurate eligibility determinations.
- Communicates program policies and expectations to clients and community in a clear and detailed manner, and confirms understanding.
- Develops and maintain relationships with partner agencies, community organizations, public assistance entities, and stakeholders to facilitate successful referrals, service delivery and continuity of care.
- Develops and maintain relationships with all departments within the organization to enable inter- agency collaboration.
- Able to accurately and effectively communicate the mission of Hope for All Gulf Coast and it’s programs, including SSVF, to community members, stakeholders, partner agencies and clients to promote awareness and understanding.
- Participates in events designed to promote awareness of homelessness and / or provision of services to homeless individuals/families.
Record Keeping:
- Maintains accurate record of each contact with a client via the HMIS and hard copy file.
- Ensures data completeness and quality rates in the HMIS are consistent with applicable HMIS Policies & Procedures and electronic records accurately reflect client’s housing status, household composition, and related client level information.
- Completes data entry in the HMIS based on the current version of the SSVF HMIS workflow.
- Ensures hard copy files include required documentation based on program Policies & Procedures and accurately reflect client’s housing status, household composition, and related client level information. Submits all vehicle fuel receipts, attached to an approved purchase order, to the Director of SSVF in accordance with Hope for All Gulf Coast Policies & Procedures
JOB REQUIREMENTS:
- Keep accurate records and reports as directed
- Participate in required training
- Attend department meetings and staff meetings
- Ability to communicate with participants
- Become familiar with agencies and procedures for mainstream benefits applications
- Become familiar with CTI Case Management Model
- Become familiar with HUD rules and regulations
- Valid Alabama Driver’s License and clean driving record.

QUALIFICATIONS AND REQUIREMENTS:
- Preferably a Bachelor’s Degree in Behavioral or Social Science or comparable field
- Good oral and written communication skills
- Ability to plan and organize self and work to meet goals
- Must have a valid driver’s license for the state of Alabama and a clean driving record
- 1 year of related experience
