What are the responsibilities and job description for the Program Supervisor position at Hope for Youth?
Hope For Youth's Nassau Prevention Services is a program of time-limited, family-oriented, supportive and rehabilitative child welfare services, which are provided to children and their families for the purposes of:
- Averting a disruption of a family that will or could result in the imminent placement of a child in foster care;
- Enabling a child who has been placed in foster care to return to his/her family at an earlier time other than would otherwise be possible; or
- Reducing the likelihood that a child who has been discharged from foster care would return to such care.
To be a successful Program Supervisor you must be ready, willing and able to do the following always:
- Meet weekly for supervision to review the service plan and progress being made
- Utilize the CANS NY as the central focus of their supervision to determine which services are working and where changes need to be made.
- Review all progress notes, psychosocial, FASP and Incident Reports completed by the case planners.
- Be assigned cases as needed.
- Attend weekly IDT meetings
- Supervise 3 Case Planners and up to 40 cases at any time for a total of up to 100 cases open within the program annually.
- Perform other resulting and necessary work to meet the needs and requirements of the clients, the program, Hope For Youth, oversight agency and any other regulatory obligations
To qualify for this job, you need to be or have a minimum:
- LMSW with experience in the child welfare system
- Providing support for other projects and locations; travel as needed
- Knowledge of child serving systems
- Experience with the child welfare system necessary
- Valid NYS Driver's License in good standing
- Physical Requirements: read/write reports and log entries, walking/standing/sitting for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, ongoing access to reliable vehicle and driving own vehicle for work
