Duties
The RN Case Manager will work under the direction of our Director of Nursing & Assistant Director of Nursing, and is responsible for providing compassionate and loving care to our Hospice patients.
You will:
- Perform initial admission assessment and recertification assessments to verify a patient's hospice qualification
- Provide and retrieve signed consent forms for patient admission to Hospice
- Communicate directly with families to schedule/discuss visits or changes in plan of care
- Develop and update plans of care to manage patient symptoms and needs
- Educate families and caregivers as necessary regarding patients plan of care, medications and changes in health
- Collaborate directly with medical director for orders requests and enter orders in EMR system
- Call in new prescriptions and refills to pharmacies and manage patients' prescription med boxes
- Perform basic wound care
- Show up on time to scheduled visits with patient
- Perform visit immediately when a patient has passed and notify all necessary parties of patient's passing
- Document visits in our EMR system in a timely fashion
- Participate in a rotating on-call schedule (including holidays and weekends)
- Attend in-person biweekly IDT meetings with staff and medical director
- Attend weekly nurse meetings in the morning as needed (virtual)
- Other duties not listed as needed or directed by ADON
Requirements for the position:
- RN Certification must be current and have good standing with State Nurse Registry
- Pass background check
- Minimum 3 years Hospice Experience
- Must have great, effective communication and interpersonal skills with team, medical director, facility staff, patients and their families
- Ability to be thorough when assessing patients during visits
- Must possess a valid CPR certification
- Ability to apply sound and good judgement when creating plan of care for patients
- Effective time management and communication skills are a must
- Complete medical documentation accurately and in a timely fashion
- Experience with technology and Electronic Medical Records
- Your own dependable means of transportation
- Valid Drivers license and auto liability insurance
Job Type: Full-time
Pay: $35.00 - $40.00 per hour
Expected hours: 24 per week
