We are seeking an experienced Hospice RN Case Manager to join our team in Round Rock/Austin. This position may share coverage of the Round Rock/North Austin/South Austin areas.

We are a growing company with offices in San Antonio, Austin, Del Rio, Dallas, Tyler and Sherman Texas. Our owners are clinical in origin and maintain that our main focus is on patients, their family, and the care they receive at this difficult time of life.
Responsibilities:
- Administer nursing care to ill, injured, or disabled patients
- Diagnose and establish patient treatment plans
- Monitor and report changes in patient symptoms or behavior
- Communicate with collaborating physicians or specialists regarding patient care
- Educate patients about health maintenance and disease prevention
- Facilitate referrals to other healthcare professionals and medical facilities
- Maintain accurate patient medical records
- Provide advice and emotional support to patients and their family members
Qualifications:
- Previous hospice experience in nursing or other medical fields
- Familiarity with medical software and equipment
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
Job Type: Full-time
Pay: $80,000.00 - $95,000.00 per year (DOE)
SIGN ON BONUS: $2,500.00
Payout Schedule -
$1,250.00 after completing successful 90 day probationary period
$1,250.00 after completing 6 months of employment
