Hope Hospice & Healthcare, Inc., a leading provider of end-of-life care, is presently seeking an energetic and passionate Admission Nurse. The Admission Nurse will handle admissions. This is a part time position, must be able to work on Mondays & Wednesdays.
General Responsibilities include:
The RN Admission Nurse serves as liaison to and conduit between the Administrator, DPCS, the patients and their families, and the community. Ensures that only those patients for whom the hospice can provide adequate care are admitted to the hospice service.
A Hospice Nurse Case Manager coordinates and manages care for terminally ill patients and their families in the home setting, focusing on comfort, quality of life, and end-of-life care. They assess patient needs, develop individualized care plans, communicate with the healthcare team, and advocate for patients and families within the complex healthcare system.
Ideal candidate background and experience:
- Current, active RN license within the state of California. Certification in Hospice and Palliative Nursing desired.
- Minimum of one-year experience as a professional nurse within the last 3 years or have a Baccalaureate in Nursing from a program accredited by the National League for Nursing.
- Demonstrated competency in performance of professional nursing functions in hospice is preferred.
- The admission nurse must possess the appropriate knowledge of applicable laws, regulations and standards.
- Excellent communication skills are required. The ability to organize, delegate, supervise and evaluate staff and understand the roles and responsibilities of the hospice interdisciplinary team is critical.
- Must possess a sympathetic attitude toward caring for the hospice patient and demonstrate positive communication skills in interacting with other members of the hospice team.
- Valid driver’s license and auto insurance.

Pay: $45.00 - $52.00 per hour
