What you’ll be doing
- Ensure advancement of appropriate admission of referrals in both residential and community divisions. Coordinate and expedite client entry, including completion of paperwork, written communication and confirmation with consumer and family, assembly of record and distribution of new consumer information to case manager.
- Participate in the implementation of annual strategic plan for Hope Network Neuro Rehabilitation Services.
- Work cooperatively with program administration to address quality concerns as stated by consumers and families, insurance representatives, stakeholders, outside professionals. Provide a positive representation of the program to the community.
- Participate in the development of an annual strategic plan for Hope Network Neuro Rehabilitation. Assist in achieving admissions goals as outlined for this plan.
- Respond professionally and positively to program inquiries, and while conducting facility tours.
- Strengthen relationships with all referral sources by acting as the initial contact, providing active listening, resources, and recommendations.
- Prepare routine and relevant program reports and narratives as requested.
- Assist in developing and securing single case agreements including obtaining documentation, collaboration, and communication to all stakeholders related to developing and securing single case agreements with multiple funders.
Anticipated Work Schedule
Monday-Friday 8:00am-5:00pm
This role requires in-office presence one day per week, with the remaining days available for remote work.
Job Requirements
- Minimum Education Required: Bachelor of Arts / Bachelor of Science.
- Two years’ experience working in rehabilitation or psychiatric program or another human services agency.
