What are the responsibilities and job description for the Linkage and Navigation Peer Supporter position at Hope Town, Inc.?
POSITION SUMMARY
Provide linkage and navigation for peers in high risk situations to support immediate connection to resources including safe housing and treatment. Provide peer support to peers referred from community partners including, but not limited to: recovery housing, law enforcement, jail, hospital, and detox settings in and around Portage County.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:
· Identify needed community services and supports and provide education & direction.
· Provide interventions, information, support, individual skills training, community referrals, and screening to peers.
· Assist in immediate resolution in high risk situations using the least restrictive interventions, including linking the participant to mental health and other emergency services.
· Be available for after hours calls to facilitate immediate linkage and navigation for persons in high risk situations.
· Regularly meet at/with community stakeholders to facilitate transfers.
· Contact supervisor or designated back-up for clarifications of procedures or assistance as needed.
· Assist peers in accessing benefits, including transportation, community resources, completion of forms and applications, and provide follow-up, as necessary.
· Work closely in collaboration with peers to engage them in the decision-making process and help them understand the benefits of assessment and treatment and accessing the appropriate level of care as indicated in the assessment and treatment plan.
· Coordinate services/care with all other professionals involved in peer’s services from Hope Town or other outside agencies and/or treatment providers.
· Complete GPRA documentation outlined by SOS 3.0 funding guidelines.
· Provide services with a trauma informed, client centered approach to care and from a peer perspective.
· Know and understand multiple pathways to recovery and assist peers in exploring this.
· Maintain a network of professional relationships with relevant service providers in the peer’s community.
· Complete paperwork and documentation requirements while maintaining quality, up-to-date records. This includes adherence to completion of documentation within 24 hours of service encounter.
· Ensures that all records and documentation meet HIPAA and OMHAS standards. Fully understands and applies all grant service requirements and documentation.
· Participate in regular supervision, team meetings, staff in services and ongoing training at Hope Town and maintains all required hours for license renewal.
· Attends organization and community meetings to provide advocacy and input for the population being served.
· Observes all rules of confidentiality as it relates to participant and service’s information, both internally and in dealing with outside individuals and/or agencies.
· Maintains professionalism.
· Additional duties as needed or assigned.
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
· GED or higher. Must possess certification from the State of Ohio to be a Peer Supporter.
· Strong writing skills, attention to detail, and computer knowledge.
· Valid Driver’s license
· Ability to establish and maintain effective working relationships with clients, employees. and the public
· Must be well organized
Job Types: Full-time, Part-time
Pay: $19.00 - $20.00 per hour
