The Hope Unity and Freedom Center is a diverse team of professionals providing psychosocial day support to male and female adults with a primary mental health diagnosis, dual diagnosis, and / or a coexisting disorder. Our work helps to decrease psychiatric hospitalization and provide stabilization of individuals with less intervention in the community. By providing daily routine and structure in a safe environment, each individual is able to achieve their best and improve their quality of life.
Position Summary
As a Mental Health Skill Builder with the Hope, Unity and Freedom (H.U.F.) Center, you are provided the opportunity to make a difference in the daily functioning of adults in need. Mental Health Skill Builders use clinical skill building techniques and interventions to support and assist adults in the home and community setting to meet the goals and outcomes designed by the individuals, the treatment teams, and you, the Mental Health Skill Builder. Mental Health Skill Builders are the primary support system for those in services with the H.U.F. Center, improving the lives of those around them daily.
Position Responsibilities :
- Provide individual skill building services to eligible individuals
- Establish and maintain positive relationships with other agencies involve in care of individual
- Maintain communication with Mental Health Skill Builder (when applicable) to ensure the overall well-being of individual
- Promote the program in a positive manner to elicit referrals and maintain census and growth of the program
- Maintain an accurate schedule of community sessions to be reviewed at any time
- Submit a copy of all Restraint Forms and Incident Reports to your direct supervisor
- Ensure that every Incident Report is completed immediately after the incident and submitted to the direct supervisor or Program Coordinator the same day
- Complete all Progress Notes using sound clinical documentation and abiding by all agency, Medicaid, and licensure standards
- Constantly monitor and document individuals’ progress towards completion of treatment goals and objectives
- Participate in applicable supervision (individual / group) with the direct supervisor
- Attend all annual mandatory agency training (Handle With Care, CPR & First Aid, Human Rights, HIPAA etc.)
- Identify service areas that require strengthening and seek out opportunities to attend training in such areas.
- During the COVID-19 pandemic, it is required that all employees wear pre-approved masks as designated by the CDC guidelines, to include surgical masks, KN95 masks, or N95 masks.

Requirements :
Role Qualifications :
Bachelor's Degree in psychology, social work, counseling or a related field from an accredited college or university.
Must currently hold one of the below :
Qualified Mental Health Professional - Adult (QMHP-A) certification
Qualified Mental Health Professional – Child (QMHP-C) certification
Qualified Mental Health Professional – Trainee (QMHP-E) certification
Qualified Paraprofessional in Mental Health (QPPMH) certification
Possess a valid driving license and maintain an acceptable driving record.
Own and operate an insurable and registered vehicle.
Available to work weekend hours as well as attend after work meetings.
Proficient knowledge of Microsoft Word and Excel.
Ability to navigate through the world wide web / internet.
Access to a working phone.
Access to a computer, printer and the world wide web / internet.
Excellent written and oral communication and interpersonal skills.
Experience in implementing innovative approaches to revenue generation and customer service delivery.
Possess creative problem solving and conflict resolution skills.
Must be able to complete and pass : a CPR / First Aid certification; Handle with Care physical and / or verbal intervention training; annual tuberculosis (TB) screening test; and clear a criminal background and central registry check.
