Summary:
The Program Assistant on the Care Navigation Team is responsible for receiving and processing service requests, scheduling appointments, updating records, and providing administrative support. The role involves working with individuals and families to connect them with necessary resources and services within their community.
Primary Duties:
- Receive and process service request calls, emails, and online submissions.
- Provide basic information about Care Navigation Services.
- Schedule follow-up appointments for Care Navigators.
- Collect and input demographic information into the electronic health record.
- Provide basic resource and referral information to individuals and families.
- Maintain and update the resource and referral library.
- Send and compile client satisfaction surveys.
- Prepare monthly and annual reports.
Qualifications:
- High School Diploma or equivalent
- Bilingual a must (English, Vietnamese, or Arabic)
- 1 -3 years of administrative experience (behavioral health or healthcare preferred)
- Strong computer and organizational skills (advanced proficiency in Microsoft Excel and Word)
- Experience with an Electronic Health Record (EHR) system is highly preferred but not required
- High degree of empathy and a desire to engage with individuals and families
Benefits:
- $46,000- $50,000 - Estimated salary depending on related experience
- Monday- Friday 8:00am - 5:30pm EST (depending on 1/2 or 1 hr. lunch break)
