Field Case Manager

Job Overview :

We are seeking a compassionate and dedicated healthcare professional to join our dynamic healthcare case management team. In this role, you will work directly in the field, assessing client needs, developing care plans, and coordinating resources to ensure optimal health outcomes. We are open to applicants with any of the following credentials : LPN, RN, CCM, CBIS, BSN, CLCP, CRRN, CLNC, MSW, OTR / L, or similar. The ideal candidate will have strong clinical skills, excellent communication abilities, and a deep commitment to advocating for the health and well-being of clients in a community setting.

Key Responsibilities :

Client Assessment :

  • Conduct thorough in-home or field-based assessments to evaluate clients' medical, social, and emotional needs.
  • Collaborate with clients, family members, and healthcare providers to develop and implement personalized care plans.

Care Coordination :

  • Facilitate access to necessary medical services, community resources, and support systems for the well-being of clients who have been involved in auto accidents or work injuries.
  • Stay informed of local healthcare services and community programs to ensure effective coordination of care.
  • Client Support and Education :

  • Provide nursing or therapeutic support, education, and emotional encouragement to clients and their families during in-home visits.
  • Empower clients to make informed healthcare decisions through clear, compassionate communication.
  • Monitoring and Evaluation :

  • Regularly monitor clients' progress in the field and adjust care plans as necessary to ensure optimal health outcomes.
  • Maintain accurate documentation of client interactions and health updates in compliance with organizational policies.
  • Interdisciplinary Collaboration :

  • Work closely with a variety of healthcare professionals, including physicians, social workers, and therapists, to provide comprehensive, client-centered care.
  • Actively participate in team meetings to discuss client progress and ensure coordinated care efforts across disciplines.
  • Advocacy and Education :

  • Advocate for clients' needs within the healthcare system and help navigate community resources to meet those needs.
  • Educate clients and families on healthcare processes, rights, and available support services.
  • Training and Development :

  • Engage in ongoing training to stay up-to-date with the latest practices and case management strategies.
  • Share best practices and insights with team members to enhance the quality of care provided to clients.
  • Qualifications :

  • Current licensure or credentials as an LPN, RN, CCM, CBIS, BSN, CLCP, CRRN, CLNC, MSW, OTR / L, or similar in the state of Michigan.
  • Minimum 2 years of nursing, case management, or related experience preferred.
  • CCM (Certified Case Manager) or CBIS (Certified Brain Injury Specialist) certification preferred.
  • Knowledge of healthcare systems and community resources relevant to client care.
  • Ability to work independently and manage time effectively in a field-based role.
  • Skills :

  • Excellent communication, interpersonal, and organizational skills.
  • Proficient in using electronic health records (EHR) and case management software.
  • Strong analytical and problem-solving abilities.
  • Ability to maintain confidentiality and handle sensitive client information with professionalism.
  • Exceptional time management skills, with the ability to manage a high volume of field-based visits.
  • Fluency in additional languages is preferred.
  • Work Schedule :

  • Required to work in the office 2 days per week; the remaining days will be spent in the field visiting clients.
  • No weekends or holidays required.
  • Compensation :

  • Hourly pay ranging from $28.00 to $35.00 per hour, based on experience and qualifications.

    Pay : $28.00 - $35.00 per hour
