Duties:
- Evaluate and care for patients with preventative guidance
- Evaluate patient medical history and conduct physical examinations
- Order and interpret diagnostic tests
- Provide treatment and management plans for patients.
- Collaborate with other healthcare professionals to coordinate patient care
- Educate patients and their families on disease prevention and management
- Offer patients additional ongoing support as applicable such as embedded Behavioral Health, Registered Dietitian, RPM, and Patient Engagement Outreach.

Qualifications
- Valid Nurse Practitioner license and certification
- Proficient in patient assessment and care
- Strong communication skills for effective patient interaction
- Knowledge of basic life support and IV insertion technique
- Experience working with diverse populations, including children and individuals with developmental disabilities

Pay: $40,000.00 - $140,000.00 per year
