Job Summary
The Client Services Coordinator is responsible for general procedures and practices relating to the management of case intakes and referrals: and for the accurate performance of job duties while assisting the Director of Client Operations and the Director of Nursing
Minimum Qualifications Preferred
At least two years experience in managing intakes and referrals
Major Job Responsibilities
- Assists with processing intakes and referrals: accepts cases, assigns case manager, assists with staffing additional employees and ancillary services.
- Assists with entering data on new patients, updates and maintains data in software application. Processes OASIS, including auditing for accuracy and completeness. Creates and reviews 485s for accuracy and completeness.
- Assists with providing nurses and recert reports and 485s
- Assists with processing orders
- Assures that laboratory orders are faxed and information is transferred to appropriate sources in order to maintain continuity of care.
- Tracks end of order and D/C paperwork.
- Performs regular chart audits for Medicaid, Non - Skilled consumers to maintain service within the State guidelines and regulations.
- Schedules PRN nursing coverage and assures that visits are covered.
- Other job-related duties as requested or required.
Knowledge, Skills and Abilities
- Knowledge of the procedures for processing intakes and referrals and the ability to manage these functions with accuracy and professionalism.
- Knowledge of the procedures for scheduling client care and the ability to assure reliability of coverage.
- Knowledge of the procedures and philosophies constituting the delivery of exceptional customer service.
- Knowledge of the appropriate and accepted practices of staff management and the ability to communicate tactfully, clearly and effectively, both orally and in writing.
- Ability to establish and maintain effective working relationships with coworkers, other departments and consumers.
- Ability to deal courteously and diplomatically with staff, clients and the general public.
- Ability to perform accurately the functions necessary to completion of job tasks.
- Ability to be trained to accurately operate equipment necessary to the function of the position including, but not limited to, computer hardware/software applications.
