Horizon Hospice & Palliative Care is fully committed to provide excellent care to the seriously ill and their families throughout Spokane County. Our Palliative Care program provides an extra layer of support to relieve the symptoms and stress of serious illness for patients who are not utilizing hospice. We help patients find solutions to the challenges of serious illness, determine their goals of care, and achieve the best possible quality of life.
The palliative nurse functions as a care coordinator for 60-70 patients. The nurse makes visits to the patient’s home to assess for changes, provide education, help with goal setting, and communicate concerns to providers. Care coordination includes collaboration with the palliative interdisciplinary team to meet patient goals of care.

Job Type: Part Time, Thursday – Friday, 8:00am to 6:30pm
Occasional participation in on-call rotation and 2 holidays a year
WORKING CONDITION AND HAZARDS:
Work includes driving personal vehicle to patients’ home and visiting or attending patients in various degrees of cleanliness and physical comfort. Varying types of weather and road conditions are encountered, along with working/driving during hours of darkness. Employee is exposed to persons in physical distress, including pain and disfigurement. Physical contact with patients may include exposure to body fluids and odors. Direct care contact with patients' places employee in the “high-risk” category for exposure to blood-borne pathogens. Difficult personalities depressed and anxious individuals, stressful situations and intense grief are frequently encountered. Working hours are set by nursing manager but may be extended depending on daily workload/patient needs PRN. Visiting schedule set by individual patient/family needs and schedule.
- Current RN or LPN license in the State of Washington
- Two years clinical/home care experience preferred
- Skilled in nursing practice, communication, and commitment to Hospice Philosophy
- Must have approved Criminal Background check
- Must successfully complete performance evaluation and a nursing skills competency evaluation during probationary period and annually thereafter.
- Valid Driver’s License and active Auto Insurance Card.
Skills needed to perform job duties include physical assessments, critical thinking, ability to work with people dealing with stressful situations, and strong communication skills. This position involves travel throughout the workday to various locations of patients including private homes, ALF, AFH and skilled nursing homes.
