This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare team at Horizon PACE will approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. Horizon has over 25 years’ experience providing care in Kentucky and is the largest provider of adult day health care services in the state. Horizon is opening 3 PACE locations in Kentucky, and we are excited to expand services to older Kentuckians. Under the supervision of the PACE Center Director, the Home Care Coordinator oversees the day-to-day operations of the Home Care program for Horizon PACE. Directs and supervises the Home Care R.N., Home Health Aide, and Scheduler. Shall assist leadership in policy formation and program development and implementation. Participates in the establishment of annual budgets. Provides supervision visits to home care staff in the field quarterly to evaluate skill and performance. Performs initial assessment, annual reassessment and change of condition assessments of participants in their home environment and coordinates plan of care with appropriate resources. When appropriate, provides health education for participants in their home as appropriate. Shall function as the liaison between the contracted home health agencies and personal care agencies and Horizon PACE. Will ensure compliance with competency requirements. Maintains the participant’s medical record with thorough documentation. Communicates participant’s changes and progress to appropriate staff.

Position Responsibilities:

· Performs initial assessments of participants in their home environment to determine their individual needs and coordinates plan of care with care-giving resources. Coordinates with the Interdisciplinary Team to develop plan of care for each participant.

· Performs annual re-assessments of all Horizon PACE participants to determine that appropriate in-home services are provided and that participants are in the least restrictive environment.

· Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in Interdisciplinary Team meetings, communicates participant changes, collaborates on plan of care decisions and coordination of 24-hour care delivery.

· Performs orientation, initial and annual competencies, supervises and evaluates performance of staff as required.

· Performs competency on appropriate day center staff as needed.

· Performs quarterly supervisor visits as needed.

· Serves as a liaison between Horizon PACE and contracted Home Health Agencies and Personal Care Agencies.

· Controls and prioritizes utilization of homecare staffing needs to reflect actual enrollee homecare needs.

· Ensures all contracted Home Health Agency staff complete initial and annual competency testing.

· Provides skilled supportive and restorative nursing care to participants as ordered, re-evaluating their plan of care and health status on an on-going basis.

· Maintains participants’ medical record and fulfills agency charting and reporting requirements.

· Monitors medication compliance administers medications and provides training in self-administration of all non-scheduled meds as needed.

· Provides health counseling to participants and their families on an individual or group basis as needed.

· Acts within the scope of his or her authority to practice

· Follows all Horizon PACE policies and procedures and applicable Occupational Safety and Health Administration (OSHA) safety guidelines.

· Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.

· Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.

· Maintains safe working environment. Follows Carolina Horizon PACE policies and procedures.

· Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.

· Performs other duties as required or requested.

Knowledge & Abilities:

  • Supports the Mission, Values, and Vision of Horizon PACE and the facility and adheres to the Corporate Compliance Code of Conduct.
  • Is knowledgeable of participant rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all participants in a safe, secure environment.
  • Supports, cooperates with and implements specific procedures and programs

· Supports and participates in common teamwork

  • Assist supervisor with various tasks to support department operations as directed; may also be assigned special projects.
  • Perform tasks which are supportive in nature to essential functions of the job, but which may be altered or re-designed depending on individual circumstances.
  • Assists participants and caregivers in filing grievances

Experience & Education:

  • Graduate of a school of professional nursing. Current Kentucky Registered Nurses License, Cardiopulmonary Resuscitation (CPR) certification required, Bachelors of Science Nursing (BSN) preferred.
  • A minimum of one year’s experience in working with the frail or elderly population preferred.
  • Two (2) years of nurse management experience in a hospital, nursing home or community-based setting preferred.

Pay: $35.00 per hour
