Full Time Social Worker
Seeking a full time Social Worker to work in the hospital. Social Worker will provide individualized services for patients/families social, emotional and economical needs in relation to the patient's plan of care and discharge plan. Position is Monday through Friday, office hours; with one day off every other week.
Key Responsibilities
Functions in the hospital, providing social services primarily for Med/Surg and Emergency Department but also responds as needed to other areas in the hospital.
Collaborates with physicians, nurses and other health care personnel to discuss the patient plan of care, therapeutic modifications, discharge planning and needs. Identifies patient/family goals and involves them in planning for discharge.
Provides basic intervention and counseling as required.
Collaborates with Case Manager RN for utilization review, acting as a liaison with insurance companies concerning patient reviews to coordinate services and payment as needed such as during absence of Case Manager RN.
Identifies and communicates recommendations for appropriate utilization of resources to medical staff, administration, and nursing staff.
Promotes interdisciplinary communication and documentation of the patient's plan of care.
Reviews discharge plan for all patients on a daily basis.
Helps coordinate transfers to other health care facilities.
Provides social services in Hospice after/when hospital social service needs are met.
Qualifications:
Education: Bachelor's degree in Sociology, Psychology, or a similar social science field. Masters Degree preferred. Social worker, or provisional license by the State Board of Examiners for Social Workers is needed and the ability to obtain/maintain a valid state driver's license.
Experience:
Minimum of one year case work experience in a healthcare facility, a social service agency, or fieldwork. Able to obtain, verify, and evaluate general and statistical information from a variety of sources. Requires written and verbal communication, organizational and human relation skills. Ability to use computers.
