Job Summary: The Medical Records Processor will play a crucial role in ensuring the accuracy and completeness of our electronic medical record system. This individual will be responsible for reviewing both external and internal medical records, inputting relevant data into our electronic database, and coordinating records from various providers. The role will focus on capturing key information related to patient diagnoses, associated medical data (e.g., imaging studies, lab results),

Duties/ Responsibilities:

Records Management:

  • Collect, organize, and maintain patient health records, both electronic and paper, ensuring completeness and accuracy.

  • Prepare medical records for new and existing patients, including scanning and filing of documentation into the electronic medical record (EMR) system.

  • Ensure all patient information is entered, updated, and stored according to clinic protocols and regulatory standards.

Release of Information:

  • Process requests for medical records from patients, healthcare providers, and legal entities while ensuring compliance with HIPAA and other privacy laws.

  • Review authorization forms for validity and obtain necessary approvals before releasing records.

  • Ensure that all records released follow clinic guidelines and are documented in the patient’s chart.

Data Integrity & Quality Control:

  • Conduct regular audits to ensure that all patient records are accurate, complete, and compliant with clinic policies and healthcare regulations.

  • Identify and resolve discrepancies or missing information in medical records.

  • Monitor and maintain the integrity of the EMR system, reporting any system issues or malfunctions.

Confidentiality & Compliance:

  • Ensure that all medical records are managed in accordance with HIPAA and other federal, state, and clinic privacy regulations.

  • Follow proper protocols for securing patient information, including access control and confidentiality agreements.

Communication & Collaboration:

  • Serve as the point of contact for physicians, nurses, and other healthcare professionals seeking access to medical records.

  • Assist patients with questions related to their medical records and explain procedures for obtaining copies.

  • Coordinate with external organizations (e.g., insurance companies, law firms) to facilitate proper and legal sharing of records.

Record Archiving:

  • Archive inactive or old medical records according to clinic policies, ensuring proper categorization and storage.

  • Oversee the secure destruction of obsolete records in line with legal retention guidelines.

  • Performs other related duties as assigned.


Education/Certifications/Licenses

Required

  • High school diploma or equivalent required.

  • Certification in Health Information Management (HIM) or Medical Records preferred.

  • Knowledge of HIPAA regulations and medical terminology is required.

Experience

  • Minimum of 1-3 years of experience working in a medical office, clinic, or health information management role.

  • Experience working with electronic medical records (EMR) systems is required.

  • Prior experience processing medical records and handling release of information requests is preferred.


Required Skills/Abilities

  • Strong organizational skills with a high degree of accuracy and attention to detail.

  • Excellent communication skills, both verbal and written, for interacting with staff, patients, and third parties.

  • Knowledge of healthcare privacy laws, including HIPAA, and the ability to handle sensitive information with discretion.

  • Proficiency in medical records software and EMR systems.

  • Ability to multitask, prioritize, and manage time efficiently.

  • Problem-solving skills, particularly in resolving discrepancies or issues related to patient records.
