Social Worker

ESSENTIAL DUTIES AND RESPONSIBILITIES

· Completes the clinical psychosocial assessment of the comprehensive assessment and other required documentation per regulations and documentation standards in a timely and comprehensive manner.

· Assist physicians and other Hospice care staff and interdisciplinary team members in understanding the significant social and emotional factors related to client physical health, mental health, and psychosocial issues.

· Develops a plan of care and interventions based on clinical psychosocial assessment in coordination with IDT/IDG members.

· Participate in case conferences with agency staff. Will be an active member of the IDT/IDG

· Prepare client clinical and progress notes, incorporating notes into the clinical record.

· Assist patients and their families with personal and environmental difficulties, which interfere with a patient’s or family’s ability to obtain maximum benefits from medical end-of-life care.

· Identifying and utilizing appropriate community resources and assessing patient/family ability to access them.

· Providing counseling services to include short-term individual counseling and crisis intervention to minimize the stress and problems that arise from the terminal illness, related conditions and the dying process. This includes individual, family and group services as indicated based on patient/family need.

· Identifying patient/family needs when discharged or when level of care changes and facilitate discharges and transfers to ensure continuity of care.

· Provides assistance with information regarding and/or completion of advance directives

· Participate in education programs and provide in-service to other Hospice personnel and community partners as requested.

· Assist clients with short-range and long-range planning and decision making.

· Assess with social and emotional factors in order to estimate the patient and patient’s family’s potential capacity and capability to cope with problems of daily living, emotional factors and end-of-life care needs.

· Assess caregiver’s ability to function adequately, including the identification of obstacles to learning or ability to participate in the care of the patient.

· Assess bereavement needs. Provide counseling services for anticipatory grief to patient’s family/support system. Facilitate referrals as needed.

· Participates in the Social Work On-Call rotation

· Assists in the referral and admission process as needed

· Will maintain competencies of end-of-life care philosophy; interventions for suicidal or homicidal issues with patients, patient’s family or significant other; admission, discharge, recertification and patient transfer methods; assessment for drug and/or alcohol use, misuse, and abuse; and abuse, neglect and exploitation issues.

· Assess hospice family to identify bereavement needs as evidenced by documentation, and community resource

Requirements

· LBSW, LMSW or LCSW issued by Texas Board of Social Work Examiners

· Bachelors Degree required. Masters, preferred.

· Previous Hospice experience, or 1 year in health care setting
