What are the responsibilities and job description for the Clinical Liaison position at Hospice Company?
Job Summary:
Primary function is to perform admissions and administer nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assume the responsibility for coordination of care. In addition to acting as clinical liaison between Agency and the community regarding community and customer education and clinical needs.
Education and License:
- Licensed Registered Nurse in the State of Texas , and
- Associates Degree in Nursing
Experience:
- Two years experience as a Registered Nurse in a clinical care setting, and
- One year of full time hospice.
Essential Functions:
- Assess hospice patients utilizing the Nursing Assessment and LCD criteria to identify the physical, psycho social, and environmental needs.
- Establishes a public relations program for interpretation of the agency’s services and to foster good working relations with physicians and community agencies.
- Develop and document the plan of care in correlation with Assessment
- Provide nursing care utilizing infection control measures (OSHA)
- Work with the Interdisciplinary Team to assure patient/family needs are met.
- Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care.
- Timely submission of all required paperwork.
- Provides counsel to the Agency regarding needs of the community; program development including personnel needs and clinical needs.
- Demonstrates an in-depth knowledge of, and ensures compliance with, all local, state and federal laws relating to marketing of the agency.
- Partners with the Interdisciplinary Team to support safe and effective patient/family care.
Skills:
- Nursing skills as defined as generally accepted standards of practice.
- Knowledge of Sate and Federal regulations.
- Good interpersonal skills.

Pay: $67,309.87 - $81,061.35 per year
