Description
Scope of Position: The Intake Coordinator is responsible for managing all clerical aspects of a patient's admission process, from initial intake to final admission, ensuring a smooth and timely experience for patients.
Essential Functions:
- Maintain professionalism in all interactions with staff, patients, and families.

Answer and triage all intake calls. Document information regarding referrals and communicate effectively to the intake team. Communicate all updates back to the referral source timely and accurately.

Responsible for completing the morning census email and updating the census boards

Review and edit the Admission Department reports timely.

Responsible for tracking referrals not yet admitted.

Assist the intake team with scanning completed checklists and referrals.

Provides backup coverage in intake as needed.

Oversees all administrative elements of the admissions process, working in coordination with the nursing and accounting departments.

Monitor all referrals that have not been admitted and participate in timely follow-up actions as needed.

Coordinate with social services/nursing/marketing to schedule information/admission visit
Qualifications
Education:
- High School Diploma or GED required; advanced education preferred.
Experience:

Prior experience in a health care environment, home care, or hospice preferred.

Prior management experience preferred.
Skills:

Exhibit a competent working knowledge in the following computer software programs: Microsoft Word and other basic Window applications.

Exhibit excellent interpersonal, organizational, and verbal communication skills.

Meet or exceed the listed qualifications in the LA Minimum Standards for Hospice.

Must be physically able to fulfill all duties of the position unassisted.
Other Requirements:

Must have good communication skills and techniques with facilities, patients, families, office staff, and physicians.
