What are the responsibilities and job description for the Clinical Admin Assistant position at Hospice Foundation Of Greater Baton Rouge?
JOB FUNCTIONS:
- Demonstrate respect, dignity, kindness, and empathy in each encounter with all patients, families, and staff in utilizing customer service skills when addressing concerns face to face or on the telephone.
- Track and maintain daily visit verification log.
- Assist CNA Coordinator with follow-ups, as needed.
- Maintain mileage tracker system and iPad visit exceptions daily.
- Maintain patient charts with all DME equipment in use with patient(s).
- Assist with all incoming office phone calls with the ability to problem-solve and resolve matters.
- Responsible for daily updates and changes with visits in Netsmart\myUnity systems.
- Maintain daily reminder report listings to ensure it is updated with new patients and reflects the removal of deceased patients.
- Initiate and process requests of DME pickups after patient deaths.
- Responsible for communicating updates to clinical teams pertaining to daily ADP list.
- Complete and document calls of new patients in discussing CNA services to be provided.
- Assist with scheduling of CNA supervisory visits and ensuring visits are completed every 14 days.
- Monitor the Perfect Visit daily to ensure visits are complete and syncd by 10 am.
- Maintain the Perfect Visit daily/weekly count per clinician to determine if standards are met.
- Maintain comprehensive working knowledge of hospice relationships with hospitals, assisted living facilities, and others.
EDUCATION:
- High School Diploma or GED required; advanced education preferred.
WORK EXPERIENCE:
- Prior experience in a health care environment, home care, or hospice preferred.
- Knowledge of medical terminology preferred.
SPECIAL SKILLS:
- Exhibit a competent working knowledge of the following computer software programs: Microsoft Word and other basic Windows applications.
- Exhibit excellent interpersonal, organizational, written, and verbal communication skills.
REQUIREMENTS:
- Must have good communication skills and techniques with facilities, patients, families, office staff, and physicians.
