Referral and Admissions Representative
PRN (as - needed)
Mostly weekday hours available
Essential Functions
Receives and responds to all incoming hospice referrals.
Creates the initial referral record in the electronic health record and starts intake process through virtual intake platform.
Schedules admission visits with patient families with goal of 4-hour response
Retrieves and uploads pertinent medical records from hospital electronic health record platforms.
Monitors open/pending referrals and conducts necessary and appropriate follow up
Verifies patient benefit information and obtains initial prior authorizations as necessary.
Answers calls from patients, families, and referral sources regarding hospice services.
Collaborates with supervisors/leadership regarding high risk and complex referrals.
Consistently conducts daily job responsibilities in a manner that maintains excellent customer service for patients, families, referral sources and staff.
Recognizes that quality improvement is a part of every staff member’s job responsibility.
Maintain confidentiality of patient information in accordance with HIPAA regulations.
Qualifications
High school diploma or GED required.
A minimum of three years’ experience in customer service or intake in a healthcare setting
Excellent communication skills (verbal and written)
Ability to multitask and prioritize in a fast-paced environment with a high level of accuracy.
Ability to work independently; self-motivated
Proficient at basic phone and computer skills
Must be comfortable answering phones with a heavy call volume.
