Nurse Case Manager Opportunity
Hospice of Holland Inc is seeking an experienced Registered Nurse to fill a Nurse Case Manager position. As a key member of our interdisciplinary team, you will play a critical role in delivering high-quality patient care and supporting patients and families through challenging times.
About the Position
This is a full-time position, offering a salary range of $35,000 - $50,000 per year, based on experience. The successful candidate will have a graduate degree in nursing, current licensure in Michigan, and at least two years of recent clinical experience, preferably in homecare or hospice settings.
Responsibilities and Qualifications
- Assess patient needs and develop individualized plans of care
- Provide direct patient care, focusing on pain management and symptom control
- Collaborate with healthcare teams to coordinate patient care
- Supervise Home Health Aides as needed
