- Complete admission and transfer processes, ensuring patients and families understand hospice services and benefits.
- Provide counseling and education on psychological, emotional, and social aspects of death and dying.
- Assist patients and families in identifying and accessing resources to meet their needs.
- Document patient interactions and activities accurately, professionally, and in a timely manner.
