Job Overview

Hospice of Holland Inc is a non-profit organization dedicated to providing exceptional care to patients and their families during end-of-life situations.

Key Responsibilities

• Assist patients and families in managing psychosocial, emotional, and resource needs related to end-of-life issues.
• Provide counseling and education on psychological, emotional, and social aspects of death and dying.
• Help patients and families identify and utilize resources to meet environmental, financial, legal, or psychological needs.

Requirements

• Master's degree in social work from a Council on Social Work Education accredited school with current Michigan Social Work licensure as a clinical social worker.
• Current Michigan driver's license and available transportation.
• Sound clinical judgment and ability to work independently.
• Knowledge of and/or experience with end-of-life care preferred.
• Effective communication skills both verbally and in writing through concise charting.
