Position Overview:
The Intake Coordinator plays a crucial role in facilitating the admission process for patients requiring hospice care. This involves receiving referrals, gathering necessary information, coordinating with the clinical team, and ensuring a smooth transition for patients and their families into the hospice program. The Intake Coordinator serves as the initial point of contact and strives to provide compassionate and efficient support throughout the intake process.
Key Responsibilities:
- Receive and process referrals for hospice services, ensuring all necessary documentation is complete and accurate.
- Gather pertinent patient information, including medical history, current condition, and care needs.
- Collaborate with the clinical team (physicians, nurses, social workers) to effectively triage referrals and schedule hospice admissions.
- Coordinate the admission process, scheduling visits, arranging for necessary equipment and supplies, and ensuring a timely transition onto hospice services for the patient.
- Communicate effectively with patients, families, referral sources, and internal staff to provide updates, answer questions, and address concerns.
- Maintain accurate and up-to-date records of all intake activities, ensuring compliance with regulatory requirements and organizational policies.
- Develop and maintain strong relationships with referral sources, including hospitals, physicians' offices, and community organizations.
- Stay informed about current hospice regulations, best practices, and industry trends.
- Contribute to the overall success of the hospice program by providing excellent customer service and supporting the interdisciplinary team.
Schedule Requirements
The Hospice Intake Coordinator is required to be on-call for hospice admissions intake needs for up to three (3) holidays per year. The on-call shift is 0830-1700 weekdays.
Required Experience:
- Minimum of one (1) year of work experience in the healthcare field.
- Experience with medical terminology and diagnoses.
