What are the responsibilities and job description for the Hospice Chaplain - Orlando position at HOSPICE OF LAKE & SUMTER?
Cornerstone Hospice is hiring a full-time Chaplain to support our patients and families at the Orlando Hospice House. The Chaplain assists, educates, and provides support to patients and families with spiritual concerns at their private resident or at a Long-term Care facility.
SCHEDULE: Monday through Friday; 8a to 4:30p (after hours reon call rotation)
JOB DUTIES/KNOWLEDGE:
- Provides Chaplain services in accordance with the patients plan of care.
- Assists the Inter-disciplinary team to understand the significance of spiritual factors related to the patients care.
- Assesses and reassesses spiritual factors in order to help the patient and family to cope with problems related to the patients life limiting illness.
- Recognizes and utilizes community and family spiritual supports to assist with the patients plan of care.
- Develops the plan of care with the Inter-disciplinary patient team and family to deal with spiritual difficulties experienced by the patient.
- Develops, prepares, and maintains individual patient care progress notes with accuracy, timeliness, and according to policies.
- Keeps abreast of Chaplain trends and knowledge. Participates with in-service programs. Attends and participates in Inter-disciplinary team meetings.
- Cooperates with appropriate staff to provide Chaplain services to patients and patient families.
- Provides education and training when requested and arranged by the Social Services Manager.
- Participates in quality improvement program.
- Participates in Hospice sponsored events.
- Takes a leadership role in Cornerstone Hospice issues and events relating to the spiritual impact of life limiting illness.
- Provides crisis intervention for patients and their families, when and where appropriate.
QUALIFICATIONS:
- Master of Divinity degree or similar graduate degree from a Seminary accredited by the American Association of Theological Seminaries, or the American Association of Theological schools, or similar accrediting body; and a minimum of one unit of CPE or equivalent (as recognized by ACPE, APC, NACC, NAJC, CPSP, CPE-International, or ICPT)
- Two years of experience as either a pastor or chaplain in Hospice or Trauma Unit.
- Knowledge and Abilities: a)Demonstrated knowledge and commitment to the Hospice philosophy of care and the Hospice team concept. b)Able to assess data reflecting the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to their spiritual needs. c)Ability to work comfortably in a non-denominational or multi-denominational environment. d)Knowledge of multi-cultural beliefs and practices and ability to work with patients and families from diverse backgrounds, including, but not limited to, race, religion, ethnicity, or sexual orientation. e)Exceptional multi-cultural people skills; pastoral care skills; and knowledge of world religions and their end-of-life rituals.
