What are the responsibilities and job description for the Lead Team Assistant - Hospice House position at HOSPICE OF LAKE & SUMTER?
Description: This position serves as the HH receptionist and provides clerical assistance to the Hospice House Staff and Manager. As the Lead HH Assistant, the position serves as the centralized schedule for all Hospice Houses. The Lead HH Assistant is also expected to serve as a Preceptor to other HH Assistants and provide assistance to other Hospice Houses in the absence of their assigned Assistant.
JOB DUTIES/KNOWLEDGE (45%)
______ 1. Serves as the HH Receptions; Handling incoming phone calls and visitors in a professionally, helpful manner.
______ 2. Assist patients and their family with their non-clinical needs.
______ 3. Maintains adequate supplies (i.e. documents, medical and DME).
______ 4. Monitors and communicates the daily census in a timely manner.
______ 5. Performs timely and accurate data entry. Ensures timely documentation is received by the HIM Dept.
______ 6. Prepares letters, documents and memos as directed.
______ 7. Assists in the creation of the IDT agenda, including lists of re-certifications and face- to-face visit dates.
______ 8. Organizes, prepares and updates IDT materials and participates in IDT as needed.
______ 9. Performs clerical duties for the HH Staff and Manager.
______10. Reports any maintenance and/or service repairs to the Facilities Dept.
______11. Performs other duties as assigned.
In addition to the above duties of the HH Assistant, the Lead will provide the following:
______ 12. Maintains a centralized schedule for all Hospice Houses; ensuring adequate staffing for all shifts.
_______13. Serves as a Preceptor to other HH Assistants. Provides initial and ongoing training as necessary.
_______14. Assist other Hospice Houses in the absence of their assigned Assistant.

______ 1. Demonstrates a desire to set and meet objectives and to find increasingly efficient ways to perform tasks.
______ 2. Completes work, care and documentation with accuracy and within agency time frames.
______ 3. Requires minimal supervision and is self-directed.
Qualifications
1. High school diploma required.
2. Minimum two years of clerical or receptionist experience, preferably in health care field.
3. Excellent Customer service skills required.
4. Advance level computer skills required.
5. Medical Records experience preferred.
6. Demonstrated knowledge of appropriate skills for communicating, especially by phone,
with individuals of all ages, especially the geriatric population.
7. Valid Florida driver’s license and required auto liability insurance.
