SUMMARY OF JOB RESPONSIBILITIES
The Spiritual Care Counselor is responsible for the provision of spiritual care services to patients / families / caregivers of Harbor Hospice.
ESSENTIAL FUNCTIONS AND DUTIES (other duties may be assigned)
Maintains the confidentiality of patient and employee information at all times, both at work and off duty, and after the termination of employment. According to the agencies Notice of Privacy Practices and HIPAA regulations.
Is knowledgeable and functional in computer applications including database, word processing, spreadsheet, and e-mail system as required for the execution of the job.
Knowledge of Hospice concept and philosophy.
Plans, develops and carries out interventions based on the spiritual assessment.
Counsels patient / family / caregiver as needed in relationship to spiritual distress, support issues and other identified spiritual difficulties according to their traditions and preferences.
Evaluates the effectiveness of planned interventions and revises plans of care accordingly.
Connects patient / family with their faith community as needed.
Documents ongoing communication with community clergy as needed.
Provides consultation, education, and support to the IDT on spiritual care upon request.
Demonstrates open communication style with patients / families.
Assists in periodic memorial services to meet the needs of staff, volunteers, and the community as assigned.
Assists in supervision of spiritual care volunteers and visiting patients / families / caregivers.
Follows standard precautions / infection control policies.

PHYSICAL DEMANDS AND WORK ENVIRONMENT
While performing the duties of this Job, the employee is regularly required to sit.
The employee is occasionally required to stand; walk and stoop, kneel, crouch, crawl.
The employee must regularly lift and / or move up to 10 pounds, frequently lift and / or move up to 25 pounds and occasionally lift and / or move up to 50 pounds.
While performing the duties of this Job, the employee is occasionally exposed to wet and / or humid conditions and outside weather conditions.
The noise level in the work environment is usually moderate.

Qualifications
Graduate of accredited seminary school of theology (B.D., M.Div. or equivalent theological degree).
Clinical Pastoral Certificate CPE
Experience working with death and dying, individuals / family / caregiver.
