Job Title: RN Patient Access Coordinator
Schedule: Monday - Friday 8:30am - 4:30pm
The RN Patient Access Coordinator plays a vital role within the Intake Department. A vital component of this position is communication and collaboration with all members of the care team to ensure a seamless transition/hand off between shifts for the purpose of continuity of care. This position will provide significant support to our Intake Nurses, RN Case Managers & Triage Nurses.
Duties and Responsibilities:
- Collaborates with case managers and discharge planners to coordinate hospice services based on the needs of the patient.
- Coordinate and verify required medical and financial documentation to ensure appropriateness for Hospice services.
- Coordinate care with providers to determine terminal diagnosis, medications, comfort pack, treatments, and other orders (such as required medical equipment) necessary for patient care and comfort.
- Complete the pre-clinical outline to prepare for admission and oversee the Plan of Care initiation.
- Maintain positive networking relationships with external providers, vendors, and other referral sources.
- When admitting patients to the program, present a comprehensive overview of the Hospice program and review the function of the interdisciplinary team. Complete and obtain consents and provide extensive communication to all necessary Hospice staff members.
- Provide education and instructions to patients and families regarding medical issues that may arise due to the patient’s terminal condition. Utilize all elements of the nursing process to provide systematic assessments focusing on pain control and symptom management.
- Guide patients/families in preparation for coping with the physical, emotional, and psychological events that may be involved in the end-of-life transition process.
- Provide timely visits in all care settings as needed during assigned shift. Complete, maintain and submit accurate and thorough clinical documentation regarding patient’s condition and care. Record pain and symptom management actions and report changes in patient’s condition or other outcomes as appropriate.
- Communicate patient needs and changes in patient’s condition to medical providers, the clinical nursing team, and other members of the IDG group. Advocate on behalf of the patient with external providers as appropriate to ensure a continuous comprehensive plan of care.
- Be adaptable with scheduling to fit the needs of the organization and the families we serve.
- Assume responsibility for your personal growth and development and maintains and upgrade professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
- Provide coverage for other clinical roles as necessary.
