Key Responsibilities
Admissions and Assessment:
- Oversee and manage the day-to-day admissions process for all patients, ensuring compliance with hospice criteria and regulations.
- Conduct comprehensive assessments for new patients, including reviewing medical history, clinical needs, and family preferences.
- Collaborate with the intake team to ensure seamless patient transitions into hospice care, providing support in case coordination and discharge planning as needed.
- Work closely with physicians, social workers, and family members to assess eligibility and appropriateness for hospice services.
- Conduct admission appointments as needed to ensure timely patient access.
- Support the inpatient clinical team in providing high-quality, compassionate care to patients within the hospice inpatient unit.
- Monitor and evaluate patient care plans, ensuring timely updates and adjustments based on patient conditions and responses to treatment.
- Provide clinical guidance and support to nursing and care teams to ensure optimal patient outcomes, comfort, and dignity in end-of-life care.
- Ensure that the hospice environment meets all clinical, safety, and regulatory standards.
- Provide RN inpatient nursing coverage as needed for open shifts.
- Supervise and mentor a team of nurses, certified aides, and clinical staff in both the inpatient and admissions settings.
- Provide ongoing education, training, and support to the clinical team to ensure high standards of care, patient safety, and compliance with relevant regulations.
- Foster a collaborative and supportive team environment, promoting open communication and effective problem-solving.
- Act as a clinical resource and advisor for staff and families, addressing any concerns related to patient care and treatment decisions.
- Ensure that all clinical documentation is accurate, timely, and in compliance with hospice regulations, including patient care records, treatment plans, and care summaries.
- Monitor and report on patient outcomes, identifying areas for improvement and implementing strategies to enhance care quality and patient satisfaction.
- Participate in audits, quality improvement initiatives, and ensure adherence to regulatory requirements.
- Collaborate with interdisciplinary teams, including physicians, chaplains, social workers, and bereavement counselors, to ensure holistic care for each patient.
- Provide clear and compassionate communication with families regarding the care plan, goals of hospice care, and end-of-life planning.
- Assist in addressing complex clinical and ethical concerns and facilitate decision-making in collaboration with patients, families, and the clinical team.
- Registered Nurse (RN) with licensure in the State of Georgia
- Certification in Hospice and Palliative Care (CHPN) preferred.
- Minimum of 2 years of experience in hospice, oncology or home health with a strong focus on inpatient care and admissions.
- Previous leadership or supervisory experience in a clinical setting, preferably within hospice or end-of-life care.
- Strong interpersonal and communication skills, with the ability to work collaboratively with a diverse team and effectively engage with patients and families.
- Compassionate, empathetic, and patient-focused attitude with the ability to manage complex and emotionally sensitive situations.
- Requires CPR certification, as well as, a drivers license and proof of insurance.
- This position may require occasional weekend or on-call hours.
