Job Description: Hospice Nurse Practitioner
We are seeking a skilled and compassionate Hospice Nurse Practitioner to join our team at Hospice of the Midwest.
Responsibilities:
- Conduct initial and ongoing assessments of patients and families to determine hospice needs.
- Develop and implement individualized care plans that address physical, emotional, and spiritual needs.
- Administer medications and treatments as prescribed by physicians.
- Communicate effectively with healthcare teams to coordinate patient care.
Qualifications:
- Graduate of an accredited school of nursing.
- Current Registered Nurse licensure in the state.
- American Heart Association CPR certification required.
- 1-2 years of recent acute care experience in an institutional setting or home care.
