Work Schedule : regular availability to work as needed; minimum requirement of two shifts per month
This position demonstrates competency in delivery of quality patient care to patients ranging in age from infants to geriatrics. The RN is responsible for implementing a plan of care in accordance with the patient’s expressed wishes and delegates care and interventions in full compliance with the Nurse Practice Act and organizational policy, procedures and standards of care
What are the essential duties day to day?
- Assesses physical, environmental / safety needs of patient.
- Develops and coordinates plan of care in collaboration with patient, significant others, and interdisciplinary care team members to focus on holistic needs of patient in order to assure satisfactory outcomes for patient.
- Makes nursing visits according to the plan of care. Follows organizational policy at time of death.
- Acts in a professional manner to collaborate with physicians, other health care providers, and interdisciplinary team members and makes timely reports of pertinent information to appropriate health care professionals.
- Accurately and timely documents nursing assessment, interventions, and updates to Plan of Care according to the nursing standards of this organization.
- Supervises nurse aides and LPNs in the home and documents supervision according to organizational policy.
- Provides caregiver with appropriate instruction to support safe care of the patient in the home and in accordance with the current plan of care
