What are the responsibilities and job description for the Homecare Hospice Aide position at Hospice of the Piedmont?
Work Schedule : regular availability to work Mon-Fri; 8 hour shifts
This position provides personal care and home management tasks at patient's residence, under the supervision of a RN Case Manager.
What are the essential duties day to day?
- Follows the nurse aide care plan to provide compassionate care to the patient in the areas of personal hygiene, elimination, feeding, and mobility.
- Reports changes in patient's condition, concerns about patient and / or family to appropriate nursing staff in a timely fashion and participates in interdisciplinary team conferences as necessary.
- Works collaboratively with the IDG and other departments in achieving patent care goals.
- Documents patient care according to agency policy and procedures; documentation is clear, complete, concise, and timely.
- Responsible for professionally managing time, people, medical supplies, and technology.
- Establishes and adheres to a timeframe for patient visits in coordination with patients, families and facility staff members.
- Maintains a clean, safe and comfortable environment for the patient.
- Teaches and / or reinforces home safety, energy conservation and sanitation.
- Provides light housekeeping tasks.
- Assists with, or provides, meal preparation and related tasks.
- Demonstrates excellent communication skills and creates successful interpersonal relationships with patients and families.
- Complies with organizational nurse aide standards.
- Participates in staff training and education programs.
