POSITION SUMMARY
The Bereavement Manager will manage a team of compassionate and evidence-based bereavement support services to grieving families and staff members. This position is responsible for the planning and implementation of grief support services, supervising a team of specialists and/or Social Workers and managing their own caseload. The Bereavement manager is a core member of the interdisciplinary plan of care.
KEY RESPONSIBILITIES
- Oversee the development, planning, and implementation of programs meant to meet the emotional needs of individuals and/or community.
- Assign cases to Social Workers/ Bereavement Specialists
- Attends and participates in interdisciplinary care planning meetings and provides guidance related to the individuals care.
- Knowledge of Medicare, Accrediting bodies, State and all other regulations applicable to Bereavement programs.
- Knowledge of community resources available to bereaved persons.
- Document assessments and treatments according to State and Federal regulations and Hospice policy.
- Ensure patient care records are appropriately maintained according to State and Federal regulations and Hospice policy.
- Responsible for own personal assigned caseload based on acuity, team members, territories, and benchmarks.
- Facilitate bereavement counseling group sessions and workshops.
- Develop treatment plans and modify plans in response to patient progress.
- Collaborate with other caregivers and organizations departments.
- Work closely with Quality to remain updated with clinical outcomes, Consumer Assessment of Healthcare Providers and Systems (CAHPS) scores, Home Health compare and PEPPER data to create action plans to maintain and/or improve scores.
- Performs other projects and duties assigned.

Pay Range: Starting at $32.53/hour (based on experience)

REQUIRED EDUCATION, CERTIFICATIONS, AND EXPERIENCE
- Bachelors in science, Social Work, or Counseling
- Licensed Social Worker in North Dakota and Minnesota (or willing to obtain dual licensure upon offer)
- 2-4 years of experience in a health care setting
- Current drivers license and auto insurance
PREFERRED QUALIFICATOINS
- Masters Degree in social work or counseling
- Experience with grief and loss counseling
