POSITION SUMMARY
Admissions and Referrals Patient Care Manager is responsible for oversight of patient care from referral through entire admission process to honor commitment of HRRV to high quality, timely provision of care. Supervises the admission nurses, nurse navigators, intake staff and documentation coordinators. Effectively coordinates daily clinical operations in collaboration with other clinical managers to achieve common goals. Promotes teamwork to generate positive results.
This position is full-time, benefit eligible, and will be based out of our Fargo office location.
KEY RESPONSIBILITIES
- Supervises and coordinates the daily operations of the admission nurses, nurse navigators, intake staff, and documentation coordinators
- Assists with admissions, training, pain and symptom management, and other topics
- Reviews patient eligibility and assists nurses in determining eligibility of patients upon admissions
- Develops and monitors the process for non-admissions, discharges, and revocations
- Completes supervisory visits with admission nurses and nurse navigators; including ride-a-longs as required
- Obtaining orders from physicians, reviewing charts, and gathering additional medical information on patients as needed
- Ensures admission algorithm is followed
- Ensures the nursing clinical guidelines for admission nurses are followed
- Promotes Heart Model standards
- Collaborates with Community Relations Director and Care Consultants to ensure education in the community and facilities is accomplished
- Collaborates with Education team in staff development
- Ensures timely and thorough completion of all employee evaluations and supporting documents (90-day evaluations and annual evaluations)
- Reviews non-admit data to look for trends and opportunities to improve process or provide education
- Participates in development and implementation of admission boot camp training
- Monitor and improve conversion rates and other admission/referral data as relevant

Pay Range: Starting at $37.27/hour (based on experience) with additional opportunities for monthly bonus based on performance and standards.

Qualifications
REQUIRED EDUCATION, CERTIFICATIONS, AND EXPERIENCE
- Current state licensure in North Dakota and Minnesota
- Current drivers license and auto insurance
- Nursing degree required; BSN preferred
- Experience in skilled nursing facility leadership, MDS (minimum data set) and hospice experience preferred
- Communication, organization, prioritization skills
- Ability to direct and develop team members
- Ability to act decisively to effectively deal with changing priorities
- Proven record of innovative clinical and operational leadership and problem solving
- Minimum of five years of clinical practice experience
- Proven ability to achieve strategic objectives
- Exceptional communication and interpersonal skills
