Position Summary
As the Patient Care Manager of the Inpatient Unit, this position is responsible for providing clinical oversight of Inpatient Hospice staff in collaboration with the Director of Clinical Operations. Maintains open communications with inpatient and home hospice staff, as well as patients, families, physicians, referral sources and vendors. Serves as a resource for all clinical staff.
Key Responsibilities
- Ensures that services provided are consistent with regulations and with Hospice of the Red River Valley policies and procedures
- Reviews appropriateness of admissions related to hospice house criteria
- Responsible for daily oversight of patient care
- Cross train home care staff to the inpatient setting and inpatient staff to home care in collaboration with PCMs
- Ensures compliance with patient care plan on a day-to-day basis
- Coordinates work assignments and schedules staff
- Chairs/Leads Interdisciplinary Team Meetings (IDG) to evaluate level of care and/or plan for discharges
- Responsible for automated medication dispensing system management: maintaining accurate user list; maintaining patient census; updating biometrics as needed; resolving discrepancies; backing up files; working with pharmacy technician on monthly reviews or as needed
- Maintains inventory from pharmacy; sending orders
- Supervises staff, including performance evaluation and corrective action
- Ensures that orientation of new clinical staff is completed and documentation is given to Human Resources
- Promotes a culture of mutual respect, integrity and serving each other
- Other duties as assigned

Qualifications
- Graduate of an accredited school of Nursing (Registered Nurse, RN)
- Hospice Care Certification
- 5-10 years experience as a Registered Nurse
- 2-5 years of hospice nurse experience
- Knowledge of hospice services and business practices
- Supervisory and/ or preceptor experience preferred
- Must be flexible and able to cover shifts when necessary
Certifications, Licenses, Registrations Required
- Current state RN licensure in North Dakota and Minnesota (or willing to obtain following employment offer)
- Current drivers license and auto insurance
