Your Responsibilities :
Manage a caseload of patients and their families, overseeing all phases of the nursing process from assessment to evaluation
Assess patient needs and involve the interdisciplinary team as necessary
Facilitate communication with attending physicians and other agencies to ensure comprehensive care
Conduct regular visits to reassess patient needs and revise care plans accordingly
Perform clinical pronouncement of death and support families during this difficult time
Maintain precise patient records and care plans
Supervise and document the care provided by Home Health Aides in accordance with guidelines
Qualifications :
Graduated from an accredited nursing program
Current RN license in the applicable state
Current CPR and First Aid certification
Knowledgeable in quality assessment and assurance procedures
