About the Job
Sentara Health Plan is seeking a Temporary RN Case Management/Care Coordinator for its Medicare Chronic Special Needs Program (C-SNP).
This is a 6-month temporary assignment working 40 hours per week, day shift, Monday through Friday, between 8:00am-5:00pm. The position is remote and only available to candidates in Virginia.
