Schedule : Monday-Friday, 8 : 30 AM - 5 : 00 PM; occasional weekends; one major or two minor holidays annually.
Job Summary :
Join our compassionate Hospice interdisciplinary team as a full-time Hospice Chaplain. In this role, you’ll provide spiritual care and support to patients and families, developing and implementing individualized care plans to meet their unique spiritual and cultural needs.
Key Responsibilities :
- Conduct spiritual assessments and collaborate with patients, families, and the Interdisciplinary Group (IDG) to create personalized care plans.
- Deliver spiritual care and counseling, including sacraments and rituals, respecting diverse faith traditions.
- Educate patients, families, and staff on end-of-life topics, ensuring compliance with CMS regulations.
- Facilitate referrals to clergy or spiritual leaders when requested.
- Offer support to staff through counseling, educational in-services, and memorial services.
- Participate in performance improvement activities and maintain up-to-date knowledge of best practices.
- Assist with orientation and training of new employees and students.
