We are seeking a PRN Spiritual Care Counselor to provide compassionate spiritual support to hospice patients and families as part of our interdisciplinary team. The schedule for this position is Monday-Friday 8 : 30-5p as needed for PTO coverage; at least 1 weekend per month; when holidays fall on the weekend only
Key Responsibilities :
- Assess and document spiritual needs of patients and families.
- Develop and implement individualized spiritual care plans.
- Provide counseling, sacraments, and rituals based on patient needs.
- Offer spiritual support to staff and facilitate memorial services.
- Coordinate referrals for clergy or religious leaders as needed.
- Educate patients, families, and facility staff on end-of-life care.
Requirements :
This is a PRN (as-needed) position with weekend and holiday commitments per agreement.
