What are the responsibilities and job description for the Spiritual Care Counselor, PRN position at TRANSITIONS LIFECARE?
We are seeking a PRN Spiritual Care Counselor to provide compassionate spiritual support to hospice patients and families as part of our interdisciplinary team. The schedule for this position is Monday-Friday 8:30-5p as needed for PTO coverage; at least 1 weekend per month; when holidays fall on the weekend only
Key Responsibilities:
- Assess and document spiritual needs of patients and families.
- Develop and implement individualized spiritual care plans.
- Provide counseling, sacraments, and rituals based on patient needs.
- Offer spiritual support to staff and facilitate memorial services.
- Coordinate referrals for clergy or religious leaders as needed.
- Educate patients, families, and facility staff on end-of-life care.
Requirements:
- MDiv, Master of Pastoral Care, or Master of Theological Studies.
- Clinical Pastoral Education (CPE) residency preferred.
- Minimum one year of experience in a health-related setting; hospice experience preferred.
- Strong communication, counseling, and documentation skills.
- Ability to travel to patient homes and facilities.
- Reliable transportation required.
This is a PRN (as-needed) position with weekend and holiday commitments per agreement.
