Job Summary
Join our compassionate Hospice interdisciplinary team as a PRN Hospice Chaplain. In this role, you’ll provide spiritual care and support to patients and families, developing and implementing individualized care plans to meet their unique spiritual and cultural needs.
Key Responsibilities
- Conduct spiritual assessments and collaborate with patients, families, and the Interdisciplinary Group (IDG) to create personalized care plans.
- Deliver spiritual care and counseling, including sacraments and rituals, respecting diverse faith traditions.
- Educate patients, families, and staff on end-of-life topics, ensuring compliance with CMS regulations.
- Facilitate referrals to clergy or spiritual leaders when requested.
- Offer support to staff through counseling, educational in-services, and memorial services.
- Participate in performance improvement activities and maintain up-to-date knowledge of best practices.
- Assist with orientation and training of new employees and students.
- Master’s degree (MDiv, Master of Pastoral Care, or Master of Theological Studies) from an accredited institution.
- Clinical Pastoral Education residency preferred or equivalent experience.
- At least one year of experience in a health-related field (hospice experience preferred).
- Proficiency in computer use and ability to learn new software.
- Reliable transportation and ability to travel within the service area.
- Physical capability to lift/carry up to 25 lbs and perform job duties in patient homes or facilities.
- Adherence to ethical and professional standards of practice.
