SUMMARY: The CAPABLE/ GAP Social Worker will older adults to deliver social service components of the CAPABLE program after completion of required CAPABLE training through Johns Hopkins University School of Nursing. CAPABLE is a home-based program to address functional opportunities and promote independence and tenure in the community with older adults. The GAP program is a home- based transitional program that helps chronically- ill older adults obtain Long Term Care Medicaid funded services, while helping to support their immediate needs. The Social Worker will be providing educational presentations in the community for older adults. The Social Worker is responsible for recruitment and coordination of the GAP program including initial screening and assessment, assistance in determining and obtaining Medicaid eligibility, communication with the CAPABLE team, referral source relationship management, and intake activity data collection. The Social Worker uses experience knowledge, judgment and skills to provide social services to patients and families in both complicated and uncomplicated situations.

Qualifications
QUALIFICATIONS: Graduate of an accredited school with Master’s degree in Social Work, has passed the State of Georgia licensing exam and can practice as a Licensed Clinical Social Worker or is on track to do so Excellent communications skills (written and verbal) and the ability to interact successfully with a diverse client population, family members and the community at-large. Working knowledge of computers, Microsoft Word, Excel or other database software preferred. One or more years’ experience working with elderly in a community-based setting required. Must have knowledge of senior community and area resources Must Preferred Qualifications: Some knowledge and/or experience with the State of Georgia- Community Waiver programs, Medicaid and Medicare.
PHYSICAL DEMANDS: Position requires moderate visual concentration for charting/documentation and computer work. Position carries a risk of exposure to infectious diseases requiring standard safety precautions. May be required to lift up to 20 lbs. with no heavy lifting require.
WORK ENVIRONMENT: This is a full-time position that works 30-40 hours work week, Monday through Friday, week with occasional requests to work additional hours as needed. This position will report to the PACE Executive Director for supervision/ GAP responsibilities and to the OT Program Manager for operational functions with CAPABLE. This position encounters stressful situations with patients and their families regarding life decisions and medical care. This position requires travel utilizing a personal vehicle and may be required to visit patients in areas that have been classified as high risk by local authorities.
