Position Overview
We are seeking a Care Management Coordinator to manage the intake and coordination of services for our Hospice, Palliative Care, and Enhanced Care Management (ECM) Programs. This role ensures smooth patient transitions, effective service delivery, and clear communication between patients, families, healthcare providers, and care teams.
Key ResponsibilitiesReferral & Intake Management
- Receive and review referrals for hospice, palliative care, and ECM, ensuring appropriateness and compliance with service criteria.
- Coordinate the intake process, including initial patient contact, insurance verification, and timely admission to appropriate programs.
Care Coordination & Communication
- Act as the primary liaison between patients, families, healthcare providers, and care teams to ensure seamless service delivery.
- Support field clinicians by maintaining open communication with patients and families.
- Receive incoming calls from patients and families, triage concerns, and consult with the Admission Nurse or the appropriate Interdisciplinary Group (IDG) member.
Order & Documentation Management
- Document all patient interactions in the EMR system to ensure accurate and up-to-date records.
- Input physician admission orders and create initial medication profiles for new patients.
Patient & Family Education
- Educate patients and families on hospice, palliative care, and ECM services, helping them understand their care options.
Collaboration with Healthcare Providers
- Work closely with physicians, discharge planners, and other healthcare professionals to facilitate smooth patient transitions.
- Coordinate care needs and equipment to ensure a comprehensive care plan is in place before admission.
Discharge & Program Transitions
- Participate in patient discharge processes, ensuring smooth transitions between care settings or into community resources.
- Coordinate patient transfers to other programs or services as needed.
Referral Paperwork & Record Management
- Manage, track, and process referral paperwork, maintaining accurate and up-to-date records in the EMR system.
Customer Service & Relationship Management
- Build and maintain positive working relationships with referral sources and healthcare providers to ensure effective service delivery.
Additional Responsibilities
- Provide informational and admission services to patients and families.
- Make referrals to volunteer and bereavement services when appropriate.
- Perform other duties as assigned in alignment with agency goals and mission.
Qualifications
- Preferred: Medical Assistant, Nursing, or clinical background.
- Experience: Prior experience in hospice, palliative care, or care management programs is highly desirable.
- Strong communication skills with the ability to engage effectively with patients, families, healthcare providers, and team members.
- Proficiency in Electronic Medical Records (EMR) systems and clinical documentation.
- Ability to manage multiple priorities independently while maintaining organization and attention to detail.
- Preferred: ICD-10 coding experience.
