About the Role
As an Enhanced Care Management (ECM) Coordinator, you will be the go-to person for patients with complex medical and social needs. Your role is all about building relationships, coordinating care, and ensuring patients receive the support they need. Whether it’s helping them navigate their care plans, working with providers, or connecting them to community resources, your work will make a real impact on their lives.
What You’ll Be Doing:
✅ Supporting and engaging patients enrolled in the ECM Program
✅ Collaborating with healthcare providers to develop and monitor personalized care plans
✅ Educating patients on self-care, health promotion, and disease prevention
✅ Visiting patients at home when needed to provide direct support
✅ Working closely with case managers, medical teams, and community partners
✅ Assisting with discharge planning and transitions of care
✅ Attending medical appointments with patients to ensure continuity of care
✅ Documenting patient interactions and tracking referrals to support services
✅ Maintaining HIPAA compliance and following all healthcare regulations

✔ Experience working in care coordination, social services, or healthcare
✔ Knowledge of community-based health resources
✔ Ability to work both independently and as part of a team
Qualifications:
Ø Medical Assistant experience preferred
Ø At least 2 years of experience in care coordination, social services, or healthcare
Ø Bilingual in English/Spanish (Preferred)
Physical Requirements:
- Ability to sit, stand, and move for extended periods
- Ability to lift up to 50 pounds or more when needed
