The Case Manager position directly supports individuals with developmental and other disabilities served in the Career and Community Access Program. The Case Manager is responsible for ensuring the implementation of the individual's programs in-house and in the community and must have experience with case management and case reviews in a relevant health care setting.

Essential Duties and Responsibilities

Case Manager Responsibilities :

  • Manages member ISP's, related documentation and represents HOPe at ISP meetings
  • Identifies and updates member goals through the ISP process
  • Manages HCSIS caseloads and keeps them up to date
  • Supervises 5-10 MSP's and Lead MSPs
  • Reviews member's goal data
  • Primary contact for parents / guardians
  • Provides occasional ratio support as needed
  • Conducts hiring for new MSPs and Lead MSPs
  • Submits Service Deliveries on Salesforce
  • Reviews and updates daily schedule for absences and / or programming adjustments.
  • Works with the Director of the CCAP Program in scheduling activities and getting feedback from members and staff on daily schedules

Direct Care Responsibilities :

  • Ensure and safeguard individuals' human dignity, rights, and liberties.
  • Demonstrate a high regard for each person and sensitivity for cultural values, expectations and goals of individuals, families, staff and others.
  • Display professional attitude and behavior, being courteous, empathetic, helpful and positive in all interpersonal interactions.
  • Work effectively and cooperatively with individuals, staff, families, community members and others; presenting self and HOPe in a positive and professional manner.
  • Focus on providing consumer satisfaction as primary goal; regularly seeking feedback and input from persons served.
  • Maintain confidentiality by discussing individual concerns only with appropriate people in private settings.
  • Attend Monthly Team Meetings
  • Complete all required trainings
  • Assisting with Activities of Daily Living as needed
  • Assist individuals physically as necessary, including, but not limited to :
  • Using the restroom
  • Lifting and / or transfering / moving individuals in and out of wheelchairs, vehicles, up from and down to a mat, etc.
  • Report daily activities and any out of the ordinary incidents to appropriate supervisory personnel (CCAP Director, Sr. Program CCAP Director, COO and / or RN).
  • Complete incident reports as necessary; to include behavioral incident reports.
  • Other Duties

    All other job related duties as assigned, included but not limited to :

  • Daily Attendance
  • Human Rights Committee
  • Events Planning
  • Fire Safety Officer
  • End of Day clean up of all program areas.
  • QUALIFICATIONS : To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and / or ability required. Each Case Manager must possess at least a high school diploma and must have experience with case management and case reviews in a relevant health care setting) along with at least 2 years of supervisory experience.

    Education / Certifications / Licensure

    Bachelor's Degree in a related field preferred

    CERTIFICATES, LICENSES, REGISTRATIONS : Valid driver's license and satisfactory driving record required.
