What are the responsibilities and job description for the Case Manager position at House of Refuge Sunnyslope, Inc.?

The Case Manager is responsible for providing direct care for the residents of House of Refuge, Sunnyslope, creating a safe and efficient working environment per company policies, procedures and best practices. The main priority is to develop a pathway of success for our residents, through effective goal setting, resourcing and accountability.
Primary responsibilities include, but are not limited to:
- Determine resident’ needs by using case management tools to complete intake interviews, evaluations and objectives.
- Monitor resident care by assessing stabilization needs and developing plans and appropriate with strategies.
- Utilize motivational interviewing skills to ensure maximum resident engagement with the program requirements that will promote lifestyle/behavior changes to achieve optimum level of program success.
- Conduct regular meetings with residents to coordinate and supply necessary resources for proper growth and development.
- Monitor and maintain progress in connection with core objectives, including relapse prevention, financial goals, heathy relationship development and successful exit strategies.
- Observe, evaluate and intervene in the case of crises.
- Maintains clients’ records by reviewing case notes, logging events and progress.
- Attend weekly department team meetings and evaluations, disseminating results and obstacles.
- Work with residents to prepare and implement successful exit strategies and objectives.
- Attend regular meetings to assist program director and support staff in evaluating residents and overall program effectiveness.
Minimum Qualifications (Knowledge, Skills, and Abilities)
- Minimum 2 years’ experience in a supervisory or management position.
- Minimum 2 years' experience in social work, behavioral mental health, and case management.
- Flexible hours and schedule.
- Must be able to work 2 evenings/ week to meet with residents for case management sessions.
- Must have excellent communication skills both written and spoken.
- Must have proficient computer skills.
- Must be able to pass a background check and drug screening.

Job Type: Full-time
Pay: $21.00 - $23.00 per hour
Expected hours: 38 – 40 per week
