Essential Responsibilities
Patient Communication
- Completes outgoing phone calls to patients after they’ve been discharged from the hospital to meet regulatory requirements and assist providers in obtaining information prior to visits.
- Contacts patients, families, and caregivers per clinician specific instruction. Outreach may include coordination of care, test results, referral information, appointment rescheduling, and documented instructions from the clinician.
- Links patient, families, and caregivers with other community resources including HCP social worker to enable continued community- based care and to enhance their quality of life.
- Assists clinicians with referrals to specialists and orders, including but not limited to home health, hospice, palliative care, diagnostic testing, medical supplies, and equipment.
- Assists Housecall Providers staff with patients on their panel, provides support to clinical team working with assigned providers.
- Works collaboratively with the primary care clinical team to ensure excellent communication and continuity of care.
- Assists clinicians with obtaining prior authorizations.
- May authorize prescription refills for non-controlled drugs if approved by clinicians or in accordance with Housecall Providers standing orders.
- Communicates with primary care clinical team regarding patient hospitalizations and clinician visit requests, or equipment needs.
- Sends chart notes, specialist referrals, visit summaries and forms for clinician signatures as requested.
- Practice within the scope of certification, as applicable and as required by law and regulation.
- Follow Housecall Providers standing orders and protocols to support clinical care of patient.
- May be required to go out into the community to collect lab specimens or administer vaccines.
- Documents all patient interaction and support activities in an appropriate and timely fashion to maintain patient’s medical record.
- Maintains patient/family confidentiality in compliance with HIPAA laws.
- Learns and provides back-up coverage for reception, care coordination, DME, and office assistant responsibilities.

Experience and/or Education
- Medical Assistant or equivalent certification
- Minimum 3 years’ experience in a medical setting (e.g., primary care, geriatrics, and/or home health/hospice field), including experience with medical records system
- Valid driver’s license, acceptable driving record and automobile liability coverage or access to an insured vehicle
Knowledge
- Advanced knowledge of medical terminology, basic patient care, and community resources
- Detailed understanding of electronic medical record systems and the ability to integrate updates to those workflows on a regular basis
- Knowledge of HIPAA privacy rules and regulations
- Ability to perform clerical functions, patient procedures, and care, and use clinical equipment, materials, and techniques within the scope of certification
- Computer skills, including working knowledge of Microsoft Office system
- Excellent customer service skills, including the ability to interact professionally, patiently, and courteously with staff, patients, families, and vendors over the phone and in person
- Strong interpersonal skills
- Ability to work both independently and collaboratively
- Ability to take initiative and utilize innovative techniques and ingenuity
- Ability to interact with various departments throughout the organization
