Summary/Objective
The Youth & Young Adult (YYA) Transitional Housing Program Manager is a managerial and administrative case management position that supports daily operations and oversight of Housing Assistance Corporation’s Transitional Housing Program (THP) for individuals aged 18-24. The THP program is a 5-bed household that provides transitional housing services for youth aged 18-24 who are seeking an interim stable solution as they work towards housing permanency. The THP manager will be responsible for oversight of the daily operations of a 5-bed transitional housing program with primarily responsibilities including (but not limited to): providing residents with case management, ensuring guidelines and policies of the program are enforced, managing daily operations of the household, screening and onboarding of residents, supporting residents in securing future stable housing, and general responsibilities related to program oversight. There are some on-call requirements involved with this position. The program manager will also help support communication and coordination of tenants residing in HAC’s Family Empowerment Program which is also located on-site. This program supports the ongoing housing stabilization of families coming out of shelter who wish to obtain self-sufficiency in their own units. Position is full-time, (35 hours/week), in-person, and located in Hyannis.
Essential Functions
- Support the screening and onboarding of new YYA transitioning into the program
- Provide daily house management to ensure all policies and procedures of the THP program are in place
- Provide housing stabilization case management services to the YYA enrolled in the program
- Responsible for on-call (emergencies only) when not on-site
- Assist with HAC’s housing stabilization Family Empowerment program by providing oversight of the FEP tenants also on-site (support with tenant documentation, ensuring tenants are adhering to terms of lease, and other responsibilities as assigned)
- Provide case manager services to Family Empowerment clients in accordance with program guidelines and responsibilities
- Other duties as assigned and in accordance with job expectations and requirements
Additional Duties
- Completes all pertinent case notes, assessments, and reporting in regards to each client
- Collaborates with outside agencies to meet the needs of clients in the program in an effort to ensure continued housing stability
- Responsible for oversight of household upkeep, cleanliness, supplies, and all safety measures are in working order
- Weekly supervision with Director of Clinical Services to provide reports on client progress and needs
- Communication with other programmatic HAC staff as needed to provide further support to enrolled YYA
- Responsible for the coordination of on-site coverage when a need arises to be absent from location
Required Qualifications, Education, and Experience
- BA in Human Services, Psychology, Criminal Justice, or other related field.
- Two years' experience with similar participants
- Previous case management experience
- Experience with housing services and programs
- Valid MA Driver’s License
Preferred Qualifications, Education, and Experience
- Master’s Degree in Psychology, Social Work, Counselor, or related field preferred.
- Five years experience with similar participants
- Prior experience with program management

Hours of Work:
- This role is salaried with a range of 57,000-62,000 dependent on experience
- Typically, 35 hours/week during a Monday through Friday work week.
