At Houston Hospice, we focus on caring, not curing. We take a team approach to care that includes physicians, nurses, social workers, chaplains, hospice aides, volunteers, and bereavement counselors. We provide hospice service wherever patients and families need us: in their home, in a long-term care facility, or in our inpatient care unit that is based in the Texas Medical Center. Our team of doctors and nurses specialize in managing pain and symptoms that are unique to life-limiting illnesses.
Summary:
Working based out of the Texas Medical Center, this position is responsible for processing all information requests and referrals for hospice services. Facilitate timely admission of the hospice patient, meet licensing, federal and state regulations relating to referrals and admissions, build and maintain strong relationship with referral and pay sources and agency staff. Obtain/submit information ensuring appropriate service reimbursement.
Duties/Responsibilities:
- Communicate accurate information in a professional manner to referral sources
- Receive and forward referrals for appropriate follow-up.
- Request/obtain appropriate information, medical records, and physician orders/ on hospice referrals.
- Participate in the QA/PI program, as requested.
- Complete necessary data entry.
- Assure compliance with regulatory bodies.
- Maintain current admission referral board.
- Submit information for verification of reimbursement source.
- Assist in the development of intake/admission policies and processes.
- Develop and promote positive relationships with IDG and other staff.
- Communicate with supervisor if unable to complete a referral and assign admission staff.
- Monitor status of referrals on an on-going basis.
Qualifications:
- Minimum one-year experience in healthcare field -OR-
- One year experience in a similar field.
- Detailed oriented.
- Knowledgeable in applicable Federal/State laws governing home health and hospice licensure
- Excellent interpersonal skills
- Excellent communication skills, both oral and written
- Excellent organizational skills, ability to handle a variety of tasks simultaneously.
- Reliable transportation; valid Texas Driver’s License and Liability insurance.
